Overview of Indicator 14- Identification & Purpose
- The State Performance Plan (SPP) describes how states are improving educational outcomes for students with disabilities and complying with the IDEA.
- Indicator 14 is one of 17 SPP Indicators that have been developed by the PA Department of Education.
- The purpose of Indicator 14 is to collect information that will inform program improvements which will result in better post-school outcomes for youth with disabilities.
Overview of Indicator 14- Post-School Outcomes
- Two year process (2023-2024 & 2024-2025 SY)
- Year One (23-24), CMs will complete the exit form for all potential leavers during the school year as part of the PaPOSMyPlan4Success Survey Completion process.
- The Exit Form now combines the former PaPOS questions with the SAAFP to simplify the process of gathering and sharing this information.
- Once the Exit Form/SAAFP is completed, the CM will then upload the document to IEPW as the SAAFP. CMs will also email the completed Exit Form to MP4S@ccaeducate.me by end of day 5/24/2024.
Which Learners are Considered a Leaver?

Any learner with an IEP who graduates with a high school diploma between 7/1/2023 & 6/30/2024 (Exception) Learner is taking a class over the summer & is anticipating passing the class and receiving their diploma.

Any learner with an IEP who drops out of school between 7/1/2023 & 6/30/2024
What is a SAAFP?
- Summary of Academic Achievement and Functional Performance.
- The LEA is required to provide this to each learner with an IEP who is exiting high school.
- Purpose of SAAFP: The SAAFP is intended to be a student-centered document that summarizes individual abilities, skills, needs, and limitations. Its purpose is to provide recommendations to learners that will support their successful transition to adult living, learning, and working.

Provides the learner with recommendations for assistance to meet post-secondary goals

For all potential 2024 graduates, the SAAFP/Exit Forms need to be emailed to MP4S@ccaeducate.me by Friday, May 24, 2024 for the June, ESY, and Summer grads. Be sure to follow these steps:
- Email the completed Exit/SAAFP document ONLY to MP4S@ccaeducate.me (by end of day on May 24th, 2024).
- Your SAAFP/Exit Document will be dated according to their graduation time (June 7, 2024).
- Upload the document to IEPW as SAAFP for the learner.
- YYYY.MM.DD.FirstNameLastNameExitGrad (2024.06.07.HeatherSoltExitGrad).
- The date of the NOREP will be dated the DATE you are sending this to the family, via your AA.
- Case Managers will notify their AA in JIRA, under the IEP ticket, that together the Exit Grad doc and Grad NOREP are ready to be sent to the family.
- Leavers before the end of the 2023-2024 school year (e.g., aging-out).
- June 7, 2024 Graduates.
- Summer Session and ESY Graduates June 7, 2024.
Keep In Mind!
- Pay close attention to who the seniors are on your caseload and when they graduate.

Contact Information
This section will be completed by the Case Manager. The information required is the learner's current address and a primary contact. *Note* Please use the learner's personal email address, NOT their CCA one. Next area to complete will be "Person conducting the interview." This will be the Case Manager's CCA contact information and primary role. The final area of "who to contact," will include the Case Manager's immediate supervisor's CCA contact information.
Summary of Student Performance
Academic Achievement: Gather information from the learner’s IEP and progress reports to summarize the learner’s abilities, skills, needs, and limitations. Include reading, math, writing, and any other related academic area Accommodations: Include the effective accommodations, modifications, assistive technology, and instructional strategies that have been utilized to assist in the learner’s educational program. *Section VI of the IEP (Specially Designed Instruction)
Recommendations: Provide recommendations and supports that will support a successful transition to adult living, learning, working, and community
Functional Performance: Provide a summary of the learner's functional performance to include their post-school goals. The Accommodations will have no long term supports. Recommend strategies to successfully achieve theor post-school goals.
Recommendations to Assist in Meeting Post-Secondary Goals
Post-Secondary Education & Training/ Employment/ Independent Living: Check only ONE box that applies to each section of the learner's post-secondary goal, according to IEP and Transition goals. Office/Agency Contact Information: Provides the learner with WHO they may contact to complete recommendations in the section, “Next Step” & "Reason for Contact"
Next Step & Reason for Contact: The recommendations should answer questions “what do I do next?” Include suggestions for accommodations to enhance access in a post-secondary environment. Include the reason for the contact.
Student Perspective
This sections helps to promote self-advocacy and the development of an understanding of their disability and the impact it may have on their post-secondary activities. Here the learner has the opportunity to reflect on what supports and services helped them to be successful during their high school experience. The learner may also discuss what supports and/or services will be needed in their future. * Case managers are encouraged to complete this section with their learners. The learner may utilize this document for their post secondary endeavors.
