How often can you have an eye exam or replace your glasses, and still receive coverage?
With UNUM’s Vision Plan, you can receive an eye exam every 12 months, frames every 24 months and lenses or contacts every 12 months! Please note that for out of network providers, there is a set allowance towards each lens. If you require retinal imaging, it is an additional cost to the eye exam of only $39! For out of network doctors, retinal imaging is not covered, so you would have to pay full price for this procedure

Through UNUM’s Vision plan, a doctor’s visit for a contact fitting is only $40, with a $130 allowance towards your contacts! Important! Please note that contact fittings with out of network doctors are not covered!
