Creating a Qualtrics Survey
Learn how to design a simple survey.

Creating the Survey
Unit 1 will step you through the basics in creating a simple survey. While there are many functions to aid in the creation of a survey, the learner will build a simple survey to give to learners to evealuate the course.
On the main dashboard of Qualtrics, click on the blue button on the bottom left labeled "Create a new project" to begin setting up your survey.

Starting Your Survey From Scratch
Here you will be presetned with many options to utilize the pre-made surveays that is privided by the Qualtrics group. But for this entire learniung module, we will be creating a survey from scratch to customize it to our needs. Let's begin.
1. Click on the Survey Button under the From Scratch title in the Create a Project area.2. Click on the glue button on the lower bottom right to Get Started working on the survey.

1. Enter the survey name needed. 2. Leave all options as default and click on the Create Project blue button.

Developing Your First Question
1. Begin by selecting the question. 2. Next, select question type on the left. For this training, choose multiple choice under question type. For additional questions, click on add new question. 3. Click in this area to change your end of survey message. 4. Save any major changes by selecting the publish button. 5. To view your progress, select the preview button.

How to Require a Response
1. Select the question you want to add a response requirement to. 2. Scroll down to response requirements 3. Select toggle switch to turn on add requirements 4. Select either force response or request response (Force response - requires the survey taker to answer a question before proceeding) (Request response - will warn survey taker they have not answered the question and ask if they would like to continue to the next question)

Publishing Your Survey
1. As before, select the publish button to save your work. (2. If you do not publish, any major changes will not show up in the final version of the survey.)

1. Select publish to confirm you would like to publish your survey. 2. You are now ready to begin collecting data!

Gathering Data With Skip and Display Logic
Definition: display and skip logic are used to determine if and when certain questions appear in your survey, based on the response of the survey participant. While the question is selected: 1. Select display logic under the question behavior tab.

3. Select in page . 4. Select the question. 5. Select the question choice that will display the next question. 6. Choose is selected or not selected. 7 Click save.

1. Determine the destination from this question by selecting from this drop down menu. 2. Determine the answer that triggers the skip logic. 3. Choose is selected or not selected 4. Confirm your choices by clicking the blue confirm button.
