Legend says that deep within the hospital a sword was forged that together with the power of 'Pressure Injury Prevention' gems (Comfort, Continence, Circulation, and Communication) could defeat any evil. Gather the gems to assemble the ultimate weapon and defeat the dragon hiding within this game.

The most common reason patient's develop pressure injuries is from not moving. That's why the best way to ensure patients stay safe and comfortable is by helping to reposition them at least every two hours...

Proper offloading tools you can use to redistribute pressure for a patient include...

Roho Cusions

Prevelon Boots

Waffle Cusions

The proper placement for these wedges is one wedge 3 inches above the sacrum and one wedge 3 inches below the sacrum. Pillows should never be used as an offloading tool when attempting to turn or reposition a patient.

Lastly, speacialty beds are another tool you can utilize to help keep patients comfortable. It's important to note that not all patients require specialty beds, and these beds are based on a patients size, care conditions or treatent requirements

Remember, prior to turning or repositioning a patient, make sure you have the right tools you need, and let an your RN Leaders know if you are missing any equipment.

In patients with incontinence and mobility issue, preventative dressings can be placed over fragile areas or wounds that are in contact with surfaces while sitting or lying down. This will act as a barrier to reduce injury from friction and shear in connection with transfers and changes of position.

Contrary to popular belief, soap and water as well as ReadyBath wipes can disrupt a patients normal ph, dry out the skin, and increase risk for injury.

Evidence based practice shows using a skin cleanser to be best for maintaining skin integrity when cleaning incontinent patients

Moisture barriers, like zinc paste, should be used to repel moisture/irritants, improve comfort,and minimize pain.

Lesson 4 - Communication GEM

Communication GEM

Escape the inferno by learning how communication impacts pressure injury prevention and move on to defeat the dragon!

Communication is important to preventing pressure injuries. PCAs can communicate with nurses to alert them of any changes to patients' skin.

Nurses can communicate with PCAs by letting them know if a patient is at high risk for pressure injury development and assist with offloading measures.

Communicating with patients and their family is also important to keeping them informed about their care, educated on risks associated with immobility, incontinence,and participatory in pressure injury prevention

Remember if a patient develops a pressure injury, anyone who cared for them within the last 48hrs is held accountable. So utilize team work and accountability to ensure patients are being properly repositioned and risks minimized.
