Lesson 1 - Comfort GEM
Comfort GEM
Watch out, a wandering skeleton is in your way! Get the comfort gem by finishing this lesson and learning how to utilize comfort to prevent pressure injuries…
Start Lesson
Where do you think you’re going, mortal?! You won’t be worthy of the Comfort Gem until you complete this lesson. Do you dare to try?
The most common reason patient's develop pressure injuries is from not moving. That's why the best way to ensure patients stay safe and comfortable is by helping to reposition them at least every two hours...
Proper offloading tools you can use to redistribute pressure for a patient include...
A frame wedges
Foams without Boarder/ Mepilex
Roho Cusions
Prevelon Boots
Waffle Cusions
The proper placement for these wedges is one wedge 3 inches above the sacrum and one wedge 3 inches below the sacrum. Pillows should never be used as an offloading tool when attempting to turn or reposition a patient.
Lastly, speacialty beds are another tool you can utilize to help keep patients comfortable. It's important to note that not all patients require specialty beds, and these beds are based on a patients size, care conditions or treatent requirements
Remember, prior to turning or repositioning a patient, make sure you have the right tools you need, and let an your RN Leaders know if you are missing any equipment.
Congratulations!! Here is your reward, you have earned it
Lesson 2 - Continence GEM
Continence GEM
Oh no! There's nothing worse than moisture to increase a patient's risk for pressure injuries. Complete the next lesson to recieve this gem...
Start Lesson
Incontinence in patients can lead to Moisture Associated Skin Damage, making them 5.1 times more likely to develop a pressure injury
Click on here to learn more
In patients with incontinence and mobility issue, preventative dressings can be placed over fragile areas or wounds that are in contact with surfaces while sitting or lying down. This will act as a barrier to reduce injury from friction and shear in connection with transfers and changes of position.
Contrary to popular belief, soap and water as well as ReadyBath wipes can disrupt a patients normal ph, dry out the skin, and increase risk for injury.
Evidence based practice shows using a skin cleanser to be best for maintaining skin integrity when cleaning incontinent patients
Moisture barriers, like zinc paste, should be used to repel moisture/irritants, improve comfort, and minimize pain.
Hooray! You got the Continence Gem!

Start Lesson
Communication is important to preventing pressure injuries. PCAs can communicate with nurses to alert them of any changes to patients' skin.
Nurses can communicate with PCAs by letting them know if a patient is at high risk for pressure injury development and assist with offloading measures.
Communicating with patients and their family is also important to keeping them informed about their care, educated on risks associated with immobility, incontinence, and participatory in pressure injury prevention
Remember if a patient develops a pressure injury, anyone who cared for them within the last 48hrs is held accountable. So utilize team work and accountability to ensure patients are being properly repositioned and risks minimized.
You got the Communication Gem!
