Take pictures of your participants completing each activity you have planned to demonstrate learning during your session. Add captions describing what teachers are doing for each activity.

Participant Sign-In Examples/Options

Part of your documentation is providing a sign-in sheet with specific information about your participants. This information includes:

• Name
• School/District
• Courses Taught
• Email

The sign-in sheet can be created in various ways. The sign-in can be on paper or participants can use a Google Form to sign-in.

• If you have a paper for the participants to sign-in on, you will need to scan and submit with the your documentation.
• If you choose to use a Google Form as your sign-in, you will submit the Google Sheets document with the information from the participants.
