Prior Written Notice (PWN)

The SSS will document the district’s proposal or refusals on a PEER PWN and provide it to the parent within 10 calendar days of the meeting.

Refer to the date of the meeting when discussing/summarizing what occurred. Use “The district proposed/refused” since it is from the district’s perspective.

Answer in detail, do not use N/A. Discuss any options, programs, etc. that were considered and rejected.

Discuss each evaluation/data used to make the decisions.

Answer in detail, do not use N/A. Discuss any additonal factors for example, medical information parent shared; attendance teachers shared, etc.

Answer in detail about what took place in the meeting (use 1 form for all, use numbers to differentiate if there are multiple proposals or refusals.
