Tenecteplase (TNK) for Ischemic Stroke

About TNK

TNK is a thrombolytic

Replacing tPA for Ischemic Strokes at UCI

TNK is administered as a single dose bolus

Location: Pyxis main med room

Dosing & Administration

  • Obtain patient weight
  • Calculate dose
  • Use a 5 mL syringe to draw up calculated dose
  • Complete Independent Double Check
  • Administer over 5 seconds
  • Flush line before and after administration with normal saline

Dosing: 0.25 mg/kg Concentration: 5mg/1mL Max dose: 25 mg (5mL)

Monitoring & Assessments

Same as tPA

Signs & symptoms to watch for

  • Angioedema, bleeding, neuro changes

Neuro assessment frequency

  • Every 15 min x 2 hrs
  • Every 30 min x 6 hrs
  • Every hour x 16 hrs
