The Spears School Greenwood Center for Online Excellence will mail ID cards to students who have inquired, three times/year.
During the 6th week of each semester (summer, fall, spring), ID cards will be mailed to students who requested one. If you are interested in obtaining an OSU photo ID, please following the directions on the next slide. You may wish to use your ID for exam proctoring services. The name printed on the OSU Card is the individual's official name as recorded in Oklahoma State University databases. Your digital photograph will be printed on the face of your OSU Card and will be stored in the OSU Card database. In an effort to comply with privacy issues, Oklahoma State University has eliminated the student SSN from the face of the card.
By the 2nd week of each semester, Please email spearsonline@okstate.edu the following, to request an OSU ID Card: Headshot photo (refer to submittal specifications on next slide) Campus ID number (CWID) First and Last Name Address the ID Card should be mailed to
Photo submission requirements:
- Submit a recent color photo of yourself only with no other visible people or objects, to spearsonline@okstate.edu.
- Photos submitted online should have been taken within the last six months to reflect cardholder's current appearance
- Photos should have a plain, solid white or off-white background
- Crop the image from just above the top of the head to the collarbone
- Ensure the image is centered. Blurry photos will not be accepted.
- Cardholder's full face must be directly facing the camera without a head tilt.
- Cardholder should have a neutral facial expression or a natural smile and eyes open.
- Do not wear a hat, sunglasses, dark glasses, non-prescription glasses with tinted lenses unless required medically (a medical certificate may be requested). Glare from glasses may be avoided by slightly tilting glasses downward.
- Do not wear wireless hands-free devices, or any other accessories that may obstruct the face or hair except for religious or medical purposes.
- Social media profile photos, glamour photos, and debutante photos should not be submitted
- The image should be a .jpg or jpeg file and should be less than 1MB.

The OSU ID Card is the property of Oklahoma State University and is non-transferable. Unauthorized use warrants confiscation and/or disciplinary action. Your card should not be surrendered to anyone. New students enrolled in credit classes are issued their first OSU Card free of charge. If this card, or subsequent cards, are lost, stolen or mutilated you will be charged for the replacement cost of a new card. It is the responsibility of the cardholder to report a lost or stolen card immediately. If your card is lost or stolen you will need to have another made with a new issue date and card number. Your OSU Card does not expire until you are no longer associated with OSU.
The Oklahoma State University ID Services office is not liable for financial loss or criminal repercussions associated with lost, stolen, damaged, or fraudulently used OSU ID Cards. Personal information is kept secure and confidential at ID Services. Outside parties are not privileged to personal or account information unless express consent is granted or the University is required to comply with legal or government agencies.
Report a lost or stolen card immediately. Do not lend your OSU Card to others.
