To enable effective care, NHS clinical systems hold records for all primary care episodes across ~6500 practices in England for 55 million patients.

Electronic records exist of all hospital episodes ('commissioning datasets') for the purposes of administration. In addition, the majority of NHS hospitals no longer use paper records. Instead, electronic software captures detailed data for all patient episodes.
