A guide to completing the ALN Referral Form

The ALN Referral Form should be used to: • Request extra support from the Specialist Team for learners with extra revision for example. This could be for the main award, essential skills or case studies and testing. • To apply for Access Arrangements for learners. • To refer a learner that has an ALN that they have not previously disclosed. • To refer a learner that you think has ALN that has not been picked up before. • To request adaptive equipment such as overlays or coloured paper. To make a referral to the Specialist Team. Fill in the Referral Form on Smart Assessor – add it like a review. You should be aiming to be as detailed as possible on the types of difficulties experienced, how they affect the learner and how they adjust to these difficulties, currently and historically. Include examples if possible, results of any practice assessments and how long they took. Some examples of types of difficulties are included below. Only include those that apply to your learner and that you can provide evidence of if required. Please ensure that you have asked the employer to complete their section so that we can gain more information and further evidence of how the difficulties impact the learner in their working environment. Once submitted the form will be sent to the Skills Manager who will apply for the access arrangements, request further information or refer to an Essential Skills Specialist for support. You will be notified of any arrangements by email. If the learner has provided you with a statement of additional learning needs, please upload to the ALS tab on Smart Assessor To see if a Referral Form has already been completed and any statement obtained for your learner by the Essential Skills Specialist, please check the ALS tab in Smart Assessor.

Reading • Needs reading support • Avoids reading out loud • Has difficulty following reading instructions • Finds it difficult to understand what they have read

Time • Finishes tasks after others • Needs extra time to finish work • Needs time to formulate an answer to a verbal question (slow responding) • Needs careful explanations in straightforward language

Organisation • Often loses things, forgets items they need to bring • Has difficulty working efficiently

Written Work • Handwriting is difficult to read • Has difficulty copying • Works on laptop rather than writing • Needs support with planning written work • Needs support with spelling • Needs support with grammar and punctuation

Memory and Concentration • Has difficulty following oral instructions • Has difficulty concentrating for long periods • Needs to have instructions repeated • Needs to have instructions written down • Has difficulty remembering what they must complete after being instructed

Practical Tasks • Prefers practical tasks to written ones • Understanding is better is able to consolidate learning through practical experience

Adjustments • What adjustments are made during visits for the learner?

Vision • Needs large print version of work • Uses coloured overlay when reading • Benefits from coloured paper
