What is COSS for?
The COSS serves as the main tool to evaluate the department-level and hospital-wide culture-building activities that are implemented through the collaboration of the Hospital Leadership, the Corporate and Clinical Leadership, Quality Management Division, Hospital License and Accreditation Division, Quality, Safety and Compliance Champions, Line heads, and staff members.

Why Conduct a Survey?
The COSS is carried out to assess the safety culture of the hospital - as a whole and as a specific unit within the hospital. It is also used as a framework for monitoring patient safety progressively and for evaluating the effect of patient safety measures.

Supplemental Items were also developed by AHRQ to assess content in areas not covered by the core questions and were also adopted in the COSS. These supplemental items cover (1) Value and Efficiency and (2) Health IT Patient Safety.

- Help hospitals assess the extent to which their organizations place a priority on and adopt practices to promote efficiency, waste reduction, patient centeredness, and high-quality care at a reasonable cost.

- Help hospitals assess the extent to which their culture is sensitive to how the use of health IT affects patient safety from the perspectives of providers and other staff working in their hospital.

Eligibility to Answer COSS
- All employees, house staff, medical staff and outsourced staff
- A respondent should have been working in MMC for more than one month

HOW TO ANSWER THE COSS

Select which respondent type is the most appropriate for you then click “Next”

Step 3
After selecting the respondent type, select the appropriate Clinical Department, MMC Division/Department or 3rd-party Company for you. Click “Next”

Select the COSS language you prefer. Click “Next”
Next
Step 5
Read the general information and instructions. Allot ample time to complete the survey in one sitting. Once done, click “Next” to proceed to survey proper.

Answer the survey!
Some of the survey questions are negatively worded to reduce the possibility of respondents just "clicking through" without actually looking into the survey's contents.
