¿COMO EMPEZAMOS?

"La reseña del paciente (especie, raza, edad y el motivo de la consulta) puede orientar hacia una predisposición de ciertas patologías. Además, es conveniente realizar un formulario de preguntas estándar para obtener la máxima información (anamnesis) así como realizar un examen físico y toma de constantes previo al examen neurológico"

OBSERVACIÓN

1.- OBSERVACIÓN

Esto nos permitirá evaluar tres elementos fundamentales:

Está controlado por la formación reticular ascendente del tronco encefálico y la corteza cerebral

Comportamiento con un estado mental normal, atento al medio.

ESTADOS MENTAL

Depresivo

Estado mental depresivo, respuesta a estímulos externos disminuidos.

Estado mental de delirio, respuesta inapropiada a estímulos externos.

ESTADOS MENTAL

Estupor

Estado mental de estupor, inconsciente responde sólo a estímulos dolorosos.

ESTADOS MENTAL

Coma

Estado mental de coma, inconsciente con ausencia de respuesta a estímulos dolorosos.

POSTURA

Está controlado por la cerebelo, recepetores sensitivos en las extremidades, sistema vestibular y vías visuales.

ANORMALIDADES EN LA POSTURA

LADEO DE CABEZA Se caracteriza por la rotación del plano medio de la cabeza. El ladeo de la cabeza suele indicar la presencia de un desorden vestibular (periférico o central). La cabeza suele estar ladeada hacia el mismo lado de la lesión (ipsilateral). Las lesiones que afecten al componente cerebelar del sistema vestibular pueden causar un síndrome vestibular central con ladeo paradójico (ladeo contralateral al lado de la lesión).

En el vídeo se observa a un paciente canino con síndrome vestibular del perro geronte.

ANORMALIDADES EN LA POSTURA

TORSIÓN- GIRO DE LA CABEZA La torsión -giro de la cabeza suele estar asociado a una torsión- giro del cuerpo (pleurotótono), esto se debe principalmente a lesiones de hemisferios cerebrales-diencéfalo. En este paciente se observa un Pleurototono izquierdo. Un Pleurototono corresponde a una desviación de la cabeza y cuello hacia un lado, esto se asocia daño en la corteza frontal del lado al cual inclina la cabeza (ipsilateral)

ANORMALIDADES EN LA POSTURA

DESVIACIONES ESPINALES Las desviaciones espinales o curvaturas pueden ser de tipo congénitas o adquiridas, y permanentes o intermitentes. Algunas lesiones frecuentes son: las hemivertebras, lesiones intramedulares que causen denervación de la musculatura paraespinal y tensión muscular, y el dolor espinal. Las curvaturas se pueden clasificar en: Escoliosis (desviación lateral), Lordosis (desviación ventral), Cifósis (curvatura dorsal), Tortícolis (torsión del cuello).

En el vídeo se aprecia la desviación de columna provocada por una escoliosis marcada, provocando incoordinación (ataxia) y movimientos espásticos de las extremidades posteriores.

ANORMALIDADES EN LA POSTURA

RIGIDEZ POR DESCEREBRACIÓN Esta postura se observa como resultado de una lesión del tronco encefálico craneal Se caracteriza por una extensión de las 4 extremidades, opistótono (extensión de la cabeza y cuello), sumado con compromiso mental (estupor o coma).

En el video se aprecia un canino con signos de descerebración (rigidez extensora y opistótonos y en estado de coma) producto de un traumatismo craneal cerrado.

ANORMALIDADES EN LA POSTURA

RIGIDEZ POR DESCEREBELACION Las lesiones ubicadas en el cerebelo resultan en opistótonos con las extremidades anteriores en extensión, las caderas pueden estar flexionadas y el estado mental se mantiene normal

ESTACIÓN DE BASE ANCHA Se caracteriza por una alteración del equilibrio, particularmente en lesiones que afectan al cerebelo y a lesiones vestibulares bilaterales

Síndrome Shiff-Sherrington

Paciente canino con Signos de Shiff-Sherrington provocado por una luxofractura vertebral en la zona toraccica.

MARCHA

Las alteraciones de la marcha se pueden deber a anomalías en la función motora o en la capacidad de coordinación (ataxia).

MOVIMIENTOS INVOLUNTARIOS O ANORMALES

Son temblores (generalizados o focalizados), mioclonías (espasmos musculares involuntarios), cataplexia (episodios súbitos de pérdida del tono muscular), crisis epileptiformes (anomalía asociadas a enfermedad epiléptica), entre otros.

2.- REACCIONES POSTURALES

"Las reacciones posturales son respuestas complejas e integradas en el cerebro. Pese a no ser pruebas capaces de proporcionar una localización precisa de la lesión, con frecuencia permiten detectar fácilmente la presencia de dicha disfunción ya que se pueden alterar significativamente".

Proporciona inervación motora a la musculatura responsable de la expresión facial y inervación sensitiva (sentido del gusto) a dos tercios rostrales de la lengua y paladar. Su componente parasimpático inerva las glándulas lagrimales y las glándulas salivares mandibular y sublingual.

El Nervio Facial se valora mediante: -Observación de la simetría facial (posición de pabellones auriculares y comisura labial, simetría palpebral). -Parpadeo espontáneo -Movimiento de narinas.

El Nervio Vestibulococlear esta involucrado en la funciones auditivas y vestibulares (adaptación de la posición ocular y corporal con respecto a la posición y movimiento de la cabeza)

La evaluación del Nervio Vestibulococlear se realiza observando la postura del cuerpo y cabeza en reposo y la evaluación de la marcha además del Reflejo vestibulo-ocular (nistagmo fisiológico)

Se debe mover la cabeza del paciente en diversas direcciones y observar el movimiento ocular en la dirección correcta.

El Síndrome vestibular se caracteriza por la presentación de nistagmos patológicos, ataxia e inclinación de la cabeza y giros hacia el lado afectado. Habitualmente son de tipo unilateral

El Nervio Glosofaríngeo da inervación motora a la musculatura de la faringe y estructuras palatinas. Su inervación sensitiva al tercio caudal de la lengua y mucosa faríngea. Su componente parasimpático inerva las glándulas salivares zigomática y parótida.

El Nervio Vago controla la función motora de la laringe, faringe y esófago y proporciona función sensitiva a la laringe, faringe y vísceras toráccicas y abdominales. Su componente parasimpático inerva vísceras toráccicas y abdominales; excepto las pélvicas.

IX par GLOSOFARINGEO - X par VAGO

Los pares IX y X se evalúan mediante: - Reflejo deglutorio. -Observar como come o bebe. -Abrir la boca ampliamente y observar lamido y deglución. Su daño provoca: disfagia, ausencia deglución, disnea, disfonía, regurgitación (megaesófago)

XI par ACCESORIO

El Nervio Accesorio proporciona inervación a los músculos trapecio y parte del esternocefálico y braquiocefálico. Su daño provoca: atrofia muscular, con desviación hacia el lado afectado en casos crónicos.

XII par HIPOGLOSO

El Nervio Hipogloso proporciona inervación motora a la musculatura lingual. Su daño provoca problemas de aprehensión, masticación y deglución.

4.- SENSIBILIDAD Y PALPACIÓN

"Se debe asegurar la capacidad del paciente de percibir dolor (nocicepción) localizando las posibles áreas de pérdida de sensibilidad. La evaluación requiere de un estímulo doloroso y la valoración de la respuesta del animal."

El dolor profundo es el dolor provocado por una pinza que genera fuerte presión sobre los huesos de los dedos. La presencia y ausencia de sensibilidad profunda es de gran valor pronóstico, especialmente en casos de lesiones espinales.

Paciente felino con ausencia de tono muscular, reflejos y dolor superficial y profundo producto de un trauma medular.

SENSIBILIDAD Y PALPACIÓN

PALPACIÓN

La palpación y manipulación sirve para detectar áreas dolorosas o reticencia al movimiento. Se suele realizar en último lugar para evitar perder la colaboración del paciente.

Paciente con presencia de hiperestesia en el área toracolumbar debido a lesión de disco intervertebral.

SÍNDROME CEREBELAR

El CEREBELO permite controlar la velocidad, proporción y la fuerza de los movimientos.

El CEREBELO se evalúa mediante: -Observación a distancia -Paso y marcha -Movimientos intencionales de cabeza y ojos -Respuesta de amenaza
